中山醫學大學附設醫院與部立南投醫院等19家地區醫院、250家診所及3家居家護理所簽定策略聯盟,將雙向轉診快捷化,每日保留600個門診名額給轉診病患,轉診中心提供掛號服務單一服務窗口,由專人辦理轉介病患,建立轉診民眾綠色通道,並提供轉診優惠,給予轉診病患完善便捷的醫療照護。
同時推動「雙主治醫師」制度,當病患轉診到醫院時,將即時發送簡訊通知診所,以利原轉介診所醫師能掌握病人就醫動態,並藉由「智慧轉診平台」與醫院主治醫師雙向溝通無縫接軌,有效連結轉診病患回到社區診所的後續醫療照護。
中國醫藥大學附設醫院結合中部地區社區診所簽約合作,成立「健康守護聯盟」,透過健保署醫療資訊雲端查詢系統與轉診平台達到即時雙向溝通,協助急重症到大型醫院,輕症回歸社區診所。
